The 23/24 Student Health Insurance Plan is offered by United Student Health Resources.

Eligibility Requirements:

• Summer or Fall Cohorts enrolled full-time in the Nurse Anesthetist Program.

Additional information regarding your policy and benefits including how to access your insurance card may be found here https://studentinsurance.fsu.edu/student-health-plans.  You will not be able to create an account until you receive confirmation that your purchase has been processed.

How to enroll:  

1. Students  will notify FSU Panama City Nurse Anesthesia Program, in writing by using the Request to Purchase Student Health Insurance and the Student Health Insurance Disclaimer. Submit the scanned, completed and signed forms via email  (no pictures/screen shots) to Geniece McPeak, gmcpeak@pc.fsu.edu no later than July 25.
2. Student will be notified of purchase within 2 weeks of the request and no later than the policy start date. 

Cancellation Requests
Cancellation requests must be submitted in writing to healthcompliance.fsu.edu.  Please review the specific conditions that are allowable for cancellations prior to submitting a request.  These can be found under Insurance FAQs.
