FSUPC Behavior of Concern

The purpose of this form is for students, staff and/or faculty (you may remain anonymous) to bring to the attention of the university’s Student Situation Response Team (SSRT) inappropriate, disruptive or threatening behavior so that appropriate actions may be taken to safeguard the campus community. If you have been the victim of such behavior or a witness to such behavior and wish to report it, this form is for that purpose. You may remain anonymous if you wish. Florida State University and the SSRT is committed to campus safety.
