Leading and advising a student organization can be as simple or as complex as the organization itself. The Department of Student Affairs provides Recognized Student Organization officers, advisors, and members a variety of resources to help them be successful. Frequently Used Forms  

Event Request
RSOs must complete an event request form for any on- or off-campus event hosted by the organization.

Must be completed at least 15 business days in advance of event date.

For events that involve physical activity and/or high risk, all participants are required to complete a waiver of liability.

Purchase Request
RSOs who have received funding from the Student Government Council (SGC) or Student Activities Board Alliance (SABA) must complete a purchase request to utilize awarded funds.

Must be completed at least 10 business days in advance of date items are needed.

Recognition Application

All new organizations and organizations apply for re-recognition each fall semester must complete this form.
